Patients with complex or catastrophic rehabilitation injuries will always have multiple clinical needs. To fully address these needs requires a clinical team and understanding aligned to the challenges facing the patient. Our typical approach involves our MDT team working with consultants, therapists, psychologists and other appropriate professionals in order to maximise functional, vocational, and social rehabilitation of the patient. We are the central point of contact, coordinating care and services across those multiple providers and settings. We work closely with individuals and their families to understand required needs and goals and develop a personalised plan, taking into consideration unique circumstances and preferences.

Medicess rehabilitation case managers are highly experienced qualified professionals from a range of disciplines including nursing, occupational therapy, physiotherapy, and psychology. Our rehabilitation case managers are members of BABICM, CMSUK and the VRA, they are also registered and regulated by either the NMC, HCPC and their own professional bodies such as RCN, RCOT, CSP, BPS.
