Find out more about the Medicess difference in rehabilitation case management for patients

Patient Portal

We have put this section of our website together to help patients understand who Medicess is, why we may be involved in your care, and what you can expect from rehabilitation case management.

You’ll find clear information about our role, how we work with you, and the standards we follow – as well as videos, downloadable resources and useful links.

Why have I been referred to Medicess?

You may have been referred to Medicess following an injury to help assess your rehabilitation needs and support your recovery.

Our role is to understand how your injury is affecting you, identify what support may help, and coordinate appropriate rehabilitation with qualified healthcare professionals.

We work with patients, solicitors, insurers and healthcare providers – but our clinical recommendations are always focused on your needs, safety and recovery.

What is rehabilitation case management?

Rehabilitation case management is a coordinated approach to recovery following injury. A dedicated rehabilitation case manager works with you to:

  • Understand your injury and how it affects your daily life
  • Identify appropriate rehabilitation support
  • Coordinate appointments and services
  • Monitor progress and adjust plans where needed

All rehabilitation is delivered by appropriately qualified, regulated healthcare professionals.

Our doctor-led, multidisciplinary approach

Medicess uses a doctor-led, multidisciplinary team approach.

This means a medical doctor oversees the clinical direction of your rehabilitation, supported by professionals such as physiotherapists, psychologists, nurses, occupational therapists and vocational specialists where appropriate.

This approach helps ensure:

  • Your rehabilitation is clinically appropriate
  • Care is coordinated rather than fragmented
  • Decisions are consistent and evidence based

Your choice, consent and independence

Your involvement with Medicess is based on your consent. You have the right to:

  • Understand why Medicess has been suggested
  • Ask questions about your care
  • Be involved in decisions about your rehabilitation
  • Raise concerns or provide feedback at any stage

Medicess is an independent rehabilitation provider. We do not have financial or referral relationships that influence our clinical recommendations.
